The Medical Records Release and/or Release of Information to a Family Member can be faxed back to us 949-248-9339. The office policies will be signed when you check in at the office. For new patients, to register, you should have received a link via email to OnPatient. Check your junk or spam folder. The link is only good for 7 days, so let us know if you need a new link. If you have trouble with the OnPatient Registration, you can complete it in the office when you arrive for your appointment.
